Hypercalcemia, also spelled hypercalcaemia, is a high calcium (ca 2+) level in the blood serum. the normal range is 2.1–2.6 mmol/l (8.8–10.7 mg/dl, 4.3–5.2 meq/l), with levels greater than 2.6 mmol/l defined as hypercalcemia. those with a mild increase that has developed slowly typically have no symptoms. in those with greater levels or rapid onset, symptoms may include abdominal pain.
When blood glucose <250mg/dl (13.8 mmol/l), change iv fluids to d5<sub 0.45%ns and reduce sc aspart insulin to 0.05 units/kg/hr; keep glucose at 150mg/dl (11 mmol/l) until resolution of dka. 2hr protocol. initial dose sc short acting insulin (e.g. aspart): 0.3 units/kg ideal body weight, followed by 0.2 units/kg sc 1 hour later followed by q2hr.
